The following documents are required to obtain FSSAI basic registration, FSSAI state license and FSSAI central license:

Documents required FSSAI Basic Registration

  • Form A duly completed and signed
  • Name , address and Contact details of Business
  • Passport size photo and address proof of Authorized person
  • Description of business
  • Bank details
  • Property Papers – (if the property is owned)
  • Rent agreement ( if rented)
  • Affidavit of proprietorship/ Partnership deed / (AOA) in case of a company
  • List of food items manufactured or processed
  • Food safety management system plan.
  • Business registration certificate if any.

Supporting documents (if required) like NOC from Municipality or local body.

Apart from the Basic documents, the following additional documents are required to obtain FSSAI state license:

  • Form B duly completed and signed
  • Blueprint/layout Plan of the processing unit showing the dimensions and operation-wise area allocation
  • List of Directors/ Partners/ Proprietor/Executive Members of Society with address, contact details, and photo ID
  • List of equipment and machinery used with the number and installed capacity (mandatory for manufacturing and processing units only).
  • Authority letter with name and address of responsible person from manufacturer
  • Water analysis report to be used in the process to confirm the portability
  • certificate obtained under Coop Act 1861/Multi-State Coop Act 2002 (in certain cases)

Apart from the Basic documents, the following additional documents are required to obtain FSSAI central license:

  • Form B duly completed and signed
  • Blueprint/layout Plan of the processing unit showing the dimensions and operation-wise area allocation
  • List of Directors/ Partners/ Proprietor/Executive Members of Society with address, contact details, and photo ID
  • List of equipment and machinery used with the number and installed capacity (mandatory for manufacturing and processing units only).
  • Authority letter with name and address of responsible person  from manufacturer
  • Water analysis report to be used in the process to confirm the portability
  • Source of raw material to be used such as milk, meat etc
  • Recall plan wherever applicable
  • Ministry of Commerce Certificate for 100% EOU
  • NOC document issued by FSSAI
  • IE code document issued by DGFT(Director General of Foreign Trade)
  • Form IX : Nomination of Persons by a Company 
  • Certificate from Ministry of Tourism (in certain cases)
  • Supporting document for proof of turnover and transportation of products
  • Declaration form
